A concussion acknowledgment is not the same as a waiver
Several states require a specific document, with specific wording, acknowledged separately — a general liability waiver does not satisfy it and folding the two together can invalidate both. Keep them as separate documents in one packet. What the wording has to say where you are is a question for your association or your attorney; we are not a law firm.

Questions
Can one parent sign for several children?
Yes, in one sitting, with each child named. The evidence records the guardian and the child separately, which is what makes it usable later.
What about a child whose parents are separated?
Either guardian can sign, and the record says which one did. If you need both, list both as signers and the document is not complete until both have.
Do waivers expire?
A waiver covers what its wording says it covers, and for an annual membership it should say the year. SignSealer does not track the date for you: next season, send the new one to your list, and whoever has not signed gets a reminder.
