Each question is checked where it is answered — a date that is on the calendar, a birthday that is not in the future, an address long enough to be one — in the same words on the signing page, on the counter tablet and in the API, and the signature waits until every required one is done.

  • Sections. Group questions under a heading, and give a section a page of its own.
  • Conditions. Show a question only when an earlier answer calls for it; a hidden question is never asked and never owed.
  • Flags. Put an answer in front of your staff when it needs a person, such as a safety box left unticked.
  • Questions first. Ask them before the document is shown, and the document after.

What a lawyer has to settle

These are the notes already written into the document, not our opinion about the law where you operate. The product refuses to send a document with one still in it, which is the point of them.

Tattoo and Piercing Consent

  1. Age and identity. I am at least eighteen, or the age this state requires for what I am asking for, and I have shown photographic identification today. I understand the studio will not proceed if I have not.
  2. What I am asking for. The design, size, spelling, placement and jewellery are as written above and as I have approved them. I have checked the spelling and the dates myself. I understand that this is permanent, that removal is expensive, incomplete and painful, and that a tattoo cannot be undone by changing my mind tomorrow.
  3. How it will look. Colours and lines settle and lighten as they heal, skin tone and scarring change how ink looks, and no artist can promise an exact match to a reference picture. Touch-ups are as set here: touch up terms.
  4. The risks. I understand that this procedure breaks the skin and that it carries risks including pain, swelling, bruising, bleeding, infection, allergic reaction to pigment, latex or metal, scarring and keloid formation, granuloma, migration or rejection of jewellery, temporary or lasting changes in sensation, and that infection can be serious. I accept those risks.
  5. My health. I have told the studio, truthfully, about anything on this list that applies to me: health disclosures — including bleeding disorders, diabetes, heart conditions, skin conditions at the site, allergies, immune conditions, pregnancy or breastfeeding, and anything I take that thins blood. I have not been drinking and am not under the influence of drugs today.
  6. Not medical advice. This studio is not a medical practice, its staff are not clinicians, and nothing said here is medical advice or a diagnosis. If something goes wrong I will see a doctor.
  7. Aftercare. I have been given aftercare instructions and I understand that healing depends mostly on what I do over the next weeks, and that the studio is not responsible for a result affected by my aftercare, sun, swimming, picking or a piercing I change too early.
  8. Consent and release. I consent to the procedure. To the extent the law where business name operates allows, I release business name and the artist from claims arising from the ordinary and disclosed risks of it, other than claims arising from their negligence or from a failure to follow the health rules that apply to them.

By signing I confirm that I have read this, had every question answered, and meant to sign it.
