Participant Waiver and Release
Between business name and guest name, on date, for activity.
- The activity and its risks. I am taking part in activity with business name. I understand that it involves risks that cannot be removed by care or instruction, including moving water, cold, weather, terrain, equipment failure, the actions of other participants, and injury up to and including death. I accept those risks for myself.
- What I agree to do. I will follow the safety briefing and the guide's instructions, wear the safety equipment provided at all times it is required, and tell the guide before we start about any condition that could affect my safety or anyone else's.
- Fitness. I confirm that I am able to take part and that I am not under the influence of alcohol or drugs.
- Release. To the extent the law where business name operates allows, I release business name, its owners, staff and guides from claims for injury, loss or damage arising from my ordinary participation, other than claims arising from their negligence or misconduct that the law does not let a release cover.
- Medical care. If I am injured, I consent to first aid and to emergency medical care, and I am responsible for its cost.
- Photographs. business name may take photographs during the activity for its own records. Use of my image for advertising needs my separate written agreement.
