
Boarding, grooming and daycare
Vet authorization for boarding, with a limit, at drop-off.
Drop-off is chaos. The form that matters is the veterinary authorisation, with a spending limit and a number that answers, and it is the one missing at two in the morning.
Where it goes wrong
Not a feature list. These are the three moments this exists for.

The authorisation with no limit on it
"Do whatever is needed" is not an authorisation anybody wants to rely on at midnight, and neither is a blank. A number, agreed in advance, is.
The emergency contact who does not answer
One number is not a plan. Two, and the vet's own, is — and it belongs on the same document as the authorisation.
The behaviour note nobody wrote down
"He is fine with other dogs" said at the counter is not a record. On the form, signed, it is.
What happens instead

Sent with the booking
Signed at home, the evening before, when they have the vet's number in front of them rather than in the car park.

The limit is a required field
A number, not a checkbox. The document cannot be sent without one, so you never have the version without it.

Signed once, found again
A regular's authorisation is on file under their name, with the date they signed it. A first-timer signs before their first day.

Clinical consent is a different thing
Authorising a boarding kennel to seek treatment is not the same as a veterinary practice taking informed consent for a procedure, and the second has requirements this is not built for. If you are a practice rather than a boarder, the simple acknowledgments here are fine and the clinical consent is not.

Can a customer update the vet's number without signing again?
They sign a new version. That sounds like friction and is the point: the number you act on at midnight should be one somebody put their name to.
What if the owner is unreachable?
That is exactly what the limit and the second contact are for, and why both are required fields rather than optional ones.
Do I need a new form for every visit?
No. Word it to cover the year, and a regular signs once; their signed copy is found by name whenever you need it.
