Each question is checked where it is answered — a date that is on the calendar, a birthday that is not in the future, an address long enough to be one — in the same words on the signing page, on the counter tablet and in the API, and the signature waits until every required one is done.

  • Sections. Group questions under a heading, and give a section a page of its own.
  • Conditions. Show a question only when an earlier answer calls for it; a hidden question is never asked and never owed.
  • Flags. Put an answer in front of your staff when it needs a person, such as a safety box left unticked.
  • Questions first. Ask them before the document is shown, and the document after.

Condition and Damage Acknowledgment

Unit unit number (equipment), handed to guest name by business name on date.

  1. Condition at handover. Recorded together at handover: condition notes. Photographs taken at handover are kept with this record.
  2. What counts as damage. Anything beyond ordinary wear: dents, cracks, tears, missing parts, water in what should be dry, and mechanical faults caused by misuse.
  3. How it is charged. Repair at cost from business name's usual supplier, or replacement at the price listed at the counter if repair is not reasonable, up to damage limit. The renter is told the amount before it is charged and may see the invoice.
  4. Return. The unit is inspected on return with the renter present where possible; anything found is recorded here on return: return notes.

By signing I confirm that the condition recorded above is accurate at handover and that I accept responsibility for damage as described.
