Chemical Service Consent and Release

- What I am asking for. The service described above, with the result we have discussed: expected result. I understand that a colour, a lightener, a relaxer, a perm or a smoothing treatment changes the structure of the hair chemically and that the change cannot simply be undone.
- History. I have told the salon truthfully what has been on my hair and when, including box colour, henna, bleach, relaxers, keratin or smoothing treatments, and anything applied at home: hair history. I understand that a treatment applied over something the salon does not know about can break hair, turn it an unintended colour, or burn the scalp.
- What cannot be promised. Hair lifts differently depending on its history, its condition, its porosity and its natural pigment. More than one session may be needed to reach the result discussed, and reaching it at all may not be possible without damage the salon will not do. The salon will tell me where the limit is and stop there.
- The risks. Chemical services can cause dryness, breakage, unwanted tone, scalp irritation and chemical burn, and allergic reaction including, rarely, a severe one. A patch test is offered where the product requires it and my decision about it is recorded here: patch test.
- My health. Allergies, scalp or skin conditions, recent surgery, medication, and whether I am pregnant or breastfeeding are written here: health notes. This salon is not a medical practice and nothing said here is medical advice.
- Redo and refund. The salon's policy on adjustments is redo policy. Telling the salon within redo window gives it the chance to put something right; a result I change elsewhere first cannot be adjusted here.
- Consent and release. I consent to the service. To the extent the law where business name operates allows, I release business name and its staff from claims arising from the ordinary and disclosed risks of it and from a result affected by history I did not disclose, other than claims arising from their negligence.
- Photographs. Whether the salon may photograph the result and where it may show it is set here: photo permission.
